The Apgar Score

The Apgar score is a quick, standardized check of a newborn's condition in the first minutes after birth. A clinician looks at five signs — color, heart rate, reflex response, muscle tone, and breathing — scores each one 0, 1, or 2, and adds them for a total from 0 to 10. It was introduced in 1953 by Dr. Virginia Apgar to give delivery teams a common, repeatable way to answer one practical question: does this baby need help right now, and is it responding to that help?

The five signs

The name APGAR is a mnemonic for the five signs (and a nod to the surname of the doctor who created it):

Reading the total

The total falls into one of three bands:

• 7–10 — reassuring. The newborn is adapting well; routine care and observation continue.
• 4–6 — moderately abnormal. The baby may need help such as stimulation or assisted ventilation, and is watched closely.
• 0–3 — low. This signals the need for immediate resuscitation.

A perfect 10 at one minute is actually uncommon, because a newborn's hands and feet often stay slightly blue for a while (acrocyanosis). That normal finding costs one point on appearance, so many perfectly healthy babies score 9 at one minute.

Why timing matters: 1 minute, 5 minutes, and beyond

The score is recorded at set times, and each reading means something different.

• The 1-minute score reflects how well the baby tolerated the birth itself.
• The 5-minute score reflects the response to any resuscitation and is the more informative of the two.
• If the 5-minute score is below 7, the assessment is repeated every 5 minutes — at 10, 15, and 20 minutes — while care continues, so the team can see the trend over time.

Recording the trajectory is more useful than any single number: a baby who climbs from 4 at one minute to 9 at five minutes is responding well.

What the Apgar score is not

This is the most important point, and it is emphasized by the American Academy of Pediatrics and the American College of Obstetricians and Gynecologists. The Apgar score describes the baby's condition at that moment and the need for resuscitation. It was not designed to predict long-term neurological outcome, and it cannot, on its own, diagnose perinatal asphyxia (oxygen deprivation around birth) or establish its cause. A single low score does not mean a baby will have lasting problems — many babies with a low early score go on to develop completely normally.

Premature babies deserve a special note. Because of physiological immaturity, a healthy preterm infant often has weaker tone, reflexes, and breathing effort, and so may score lower than a healthy full-term baby without anything being wrong. For that reason the score is always read in the context of gestational age, not in isolation.

A worked example

Suppose a full-term newborn at one minute is pink on the body but with blue hands and feet (Appearance 1), has a heart rate of 120 bpm (Pulse 2), cries when stimulated (Grimace 2), moves actively (Activity 2), and has a strong cry (Respiration 2). The total is 1 + 2 + 2 + 2 + 2 = 9 — a reassuring score, with the single missing point explained by normal acrocyanosis.
