Corrected Calcium

Corrected calcium adjusts a measured total serum calcium for the patient's albumin level. Because much of the calcium in blood is carried by albumin, a low albumin pulls down the total reading even when the physiologically active calcium is normal. This calculator applies the Payne correction in either conventional (mg/dL) or SI (mmol/L) units.

Why the correction exists

Roughly 40% of blood calcium is bound to albumin and is not metabolically active; the rest circulates as free (ionised) calcium plus a small complexed fraction. Routine panels report total calcium, which sums all of these. When albumin falls — common in malnutrition, liver disease, or acute illness — the bound portion shrinks and the total reading drops, but the active calcium may be unchanged. Without correction, a normal patient can appear hypocalcaemic.

The formula

In SI units, with calcium in mmol/L and albumin in g/L:

Each 1 g/L that albumin sits below the reference value of 40 is estimated to mask about 0.02 mmol/L of calcium.

Worked example

For a measured calcium of 2.0 mmol/L and an albumin of 30 g/L:

Why does calcium need correcting for albumin?

About 40% of the calcium in blood is bound to albumin and is not physiologically active. Standard laboratory tests report total calcium, which includes that bound fraction. When albumin is low, the total reading falls even though the active (ionised) calcium may be normal.

The correction estimates what the total would be at a normal albumin, reducing the chance of misreading a low total as true hypocalcaemia.

When is the correction reliable?

The Payne formula works reasonably well for mild to moderate changes in albumin in stable patients. It is less reliable in critical illness, acid-base disturbances, and kidney disease, where the relationship between albumin and calcium binding shifts. In those settings a direct ionised calcium measurement is preferred over any correction.
