Cholesterol Ratio

The cholesterol ratio divides total cholesterol by HDL cholesterol. It takes two numbers from a standard lipid panel and combines them into a single figure that reflects how the protective HDL fraction compares with the total amount of cholesterol carried in the blood. As a screening number it tends to track cardiovascular risk more closely than total cholesterol on its own, because it accounts for the part of the total that is thought to be protective.

What the ratio represents

Total cholesterol is the sum of several fractions, including LDL, HDL, and a contribution from triglyceride-rich particles. HDL is the fraction associated with carrying cholesterol away from artery walls. Dividing the total by HDL therefore asks how many units of overall cholesterol there are for each unit of the protective kind. A smaller answer means HDL makes up a larger share of the total, which is generally the more favourable position.

The formula

Here $\mathrm{TC}$ is total cholesterol and $\mathrm{HDL}$ is HDL cholesterol. Both are concentrations, so they share the same unit, and that unit cancels in the division. The result is a plain number with no unit attached. This is why the figures may be entered in either mg/dL or mmol/L: the ratio comes out the same as long as both numbers use the same unit.

Worked example

Consider a lipid panel reporting a total cholesterol of two hundred and an HDL of fifty, in the same unit:

The ratio is four. The same two values expressed in mmol/L — roughly five point two over one point three — give the same figure, because the unit cancels either way. A ratio of four sits between the optimal and elevated thresholds described below.

Interpreting the number

American Heart Association guidance treats a ratio below 5 as the goal, with a value below 3.5 considered optimal. The bands shown alongside the result follow those thresholds: optimal below 3.5, moderate from 3.5 to 5, and high above 5. A high ratio can arise from a raised total, a low HDL, or both, so the underlying lipid values are worth reading together with the ratio rather than in isolation.

Limits

The ratio is a screening figure, not a diagnosis, and it does not replace the individual numbers on a lipid panel.
